General Office Policies

 

APPOINTMENTS  

All visits are by appointment only. If you are a new patient to our practice, once the appointment is scheduled, a packet of information will be sent to you that includes a patient information sheet and health questionnaire. Please fill out and bring these completed forms to your first appointment.

Your child’s initial visit to the specialist will be scheduled as a consultation and should last approximately one hour. You will receive an automated appointment reminder via telephone prior to the scheduled visit reminding you of the appointment.

We ask that you arrive at least 15 minutes prior to your child’s scheduled appointment to allow time for registration. If for some reason you need to cancel or re-schedule your appointment, we ask that you give us a minimum 24 hour notice.
 

INSURANCE/CO-PAYS  

Many insurance plans require a referral or pre-authorization in order to see a specialist. Once your child’s appointment is scheduled, we will request the referral for you from your child’s primary care physician. Please know that if we are unable to obtain a referral, your child’s appointment may need to be rescheduled. If you have already obtained your referral, please let us know.

All copays are due upon check-in. If you are experiencing a financial hardship and require financial assistance, please let the secretary know and you will be referred for financial counseling.

 

WHAT TO BRING TO YOUR CHILD’S VISIT  

It is extremely important that the specialist be aware of any medications your child may be taking. If you are unable to bring the medications to the office, please come prepared with a written list that includes dose and frequency of the medication. This list should be given to the nursing staff.

Please bring any medical records or other information that you feel may be important to share with the specialist.
